How to Create a Feedback Form With a QR Code (Free, 6 Steps)

By the Qwenvo team · · 4 min read

To create a feedback form with a QR code, build a short form, publish it, copy the link from the Share tab, paste it into any free QR generator, download the image and print it. Qwenvo does not generate QR codes itself, so the generator is a separate, quick step.

Why a QR code suits feedback

Feedback is best asked for in the moment: at the table, at the counter, at the end of a class. A printed code lets people answer on their own phone right then, without typing an address. The form has to be short and mobile-friendly, because people are standing or waiting.

Step 1: Build a form for a phone in a hurry

  1. Click New form and title it: 'How did we do today?'
  2. Type / and add a Rating, with the question 'How was your visit?'
  3. Add one Long answer: 'Anything we should know?' Make it optional.
  4. Add an optional Email if you want to reply: 'Want us to follow up? Leave your email.'
  5. Add a Thank-you page: 'Thank you.' Keep it light.

Three questions at most. For the full process, see how to create a feedback form, or start from the customer feedback form or local business review request template.

Step 2: Record where each reply came from

This is the trick that makes a QR code useful: make a different code for each place. Use a Hidden fields block with a field named location, and add it to the link: ?location=table-4. Make one code per table, desk, branch or event. The value is saved with every reply, so you can see which spot gets the complaints. Details are in hidden fields.

Step 3: Publish and copy the link

Click Publish. On the Share tab, copy the public link. If you have a custom domain, choose it for a shorter, branded address, which also looks better printed under the code.

Step 4: Make the QR code

  1. Open any free QR code generator and choose the URL or link type.
  2. Paste the link with the ?location= part for the first spot.
  3. Download the image in a high-resolution or vector format so it prints sharply.
  4. Repeat for each location.

Pick a generator that makes static codes, which encode the link directly, so they keep working without depending on the generator's service. Read its terms before relying on it.

Step 5: Print it well

  • Size. Large enough to scan from where people will be. Test it.
  • Contrast. Dark code on a light background. Do not put it on a busy photo.
  • A call to action. 'Scan to tell us how we did, 30 seconds.' A code with no explanation gets ignored.
  • A printed fallback link. Short text under the code, for people who cannot scan.
  • Placement. At eye level, where people wait, not on the floor or behind glass.

Step 6: Test, then watch the replies

  1. Scan each printed code with at least two different phones.
  2. Submit a test answer and check it in Submissions, with the right location in the column.
  3. Delete the test row.
  4. Turn on Triage submissions with AI and a routing rule, so urgent replies reach your phone by email or Slack. See AI form triage.
  5. Read the replies every week. Fix one thing and say so on the sign.

Mistakes to avoid

  • A form that is too long for someone standing up.
  • One code for everything, so you cannot tell where a reply came from.
  • Skipping the scan test after printing. A smudged or tiny code costs you answers.
  • Changing the form link later. A printed code points at one address. If you change the form's URL or domain, the old signs break. Edit the form instead of replacing it.

Honest limit: there is no built-in QR code feature in Qwenvo, and we will not pretend otherwise. If you want one app that does both, you may prefer a tool that generates them; if you are happy with a free generator, the extra step is small. For a version aimed at events, see registration forms with a QR code.

Questions people ask

Does a QR code expire?

A static code only holds a web address, so it works for as long as that address works. Publish the form on a stable link or your own domain.

Can people give feedback without an account?

Yes. They scan, answer and submit.

Can I make the feedback anonymous?

Yes, if you skip the email field. See how to make an anonymous survey.

Is this free?

The form is free and unlimited within fair use. Many QR generators are free too; check each one's terms.

Every feature, $0, no card.
