How to Create a Feedback Form That People Actually Finish

By the Qwenvo team · · 3 min read

To create a feedback form, pick the moment you want feedback on, ask a rating, then ask what worked and what to improve, and send urgent replies to someone who can act. In Qwenvo that is a form of three to five questions, built by typing /, then published and shared as a link.

Step 1: Decide what you are collecting feedback on

'Feedback' is too broad. Choose one: a purchase, a support chat, an event, a class, a product feature, your website. Write it in the form title, such as 'How was your visit today?'. A narrow form gets specific answers.

Step 2: Write three core questions

  1. A rating. Use the Rating or Linear scale block. 'How was your experience today?' Label the ends.
  2. What worked. A Long answer: 'What went well?'
  3. What to fix. A Long answer: 'What is one thing we could do better?'

Then add one optional question if you will use it, such as 'Can we contact you about your feedback?' with an Email block that appears only if they answer Yes.

Step 3: Ask different follow-ups for low and high scores

Use conditional logic so the form responds to the rating. Add a Conditional logic block (or Add conditional logic from the question's menu) and set the rules:

  • When the rating is 1 to 3, show 'We are sorry. What went wrong?' and the optional contact question.
  • When the rating is 4 to 5, show 'What did we get right?' and, if you wish, a link to leave a public review.

The rules are covered in conditional logic. Keep the form to one page unless it needs more.

Step 4: Build it

  1. Click New form, type the title and press Enter.
  2. Type / and add your rating, then the two Long answer questions.
  3. Add the logic from step 3.
  4. Add a Thank-you page: 'Thank you. We read every reply.' If you ask for public reviews, put the review link on the thank-you page, or use redirect on completion.
  5. In Settings, switch on Self email notifications so you get a message per submission, or use the routing in step 5.
  6. Click Preview, then Publish, and share the link from the Share tab.

Prefer a head start? Open the customer feedback form, website feedback or event feedback survey template and edit it. You can also describe the form to the AI builder.

Step 5: Route the urgent replies

Feedback is only useful if someone sees the angry reply while it is still fixable. Turn on Triage submissions with AI in Settings. Each response then gets a category, a priority from low to urgent, a sentiment and a one-to-two-sentence summary with a suggested reply, up to 10,000 submissions a month. Add a routing rule that sends high and urgent priorities to up to five email addresses or a Slack incoming webhook. The full setup is in AI form triage.

Where to put the form

  • A link in a follow-up email sent the same day.
  • Your website, as a Standard embed or a Popup, see embed your form.
  • A thank-you or receipt page.
  • Printed on a card or sign, using a code you make from the link. See feedback form with a QR code.

Mistakes to avoid

  • Asking too much. Each extra question lowers the chance people finish. Test the length on your own audience.
  • Requiring every field. Require the rating only.
  • Never replying. If someone leaves contact details, answer them.
  • Averaging a score you cannot act on. Read the text, not just the number.
  • Asking a leading question. 'How great was our service?' guarantees nice scores and useless data.

An honest limit: Qwenvo shows you the feedback, but it does not send automatic replies to angry customers on your behalf. It suggests a reply; you send it.
