How to Make an Anonymous Survey (and What Anonymous Really Means)

By the Qwenvo team · · 4 min read

To make an anonymous survey, do not ask for names or emails, leave the Metadata fields setting off so IP addresses and respondent ids are not stored, skip email notifications to respondents, and avoid questions that identify people in small groups. Then tell respondents plainly what you collect and what you do not.

What 'anonymous' really means

A survey is anonymous when you cannot link an answer to a person, not just when you promise not to look. That depends on three things: what the form collects, what the tool stores, and whether the answers themselves give someone away. You control the first and third. This guide covers all three.

Step 1: Build the form without identifiers

  1. Click New form and title it for the topic.
  2. Type / and add your questions. Use Multiple choice, Rating, Linear scale and Long answer.
  3. Do not add name, email, phone or employee number fields.
  4. Open Settings and check the Submissions section. Leave Metadata fields off, because that setting stores the respondent's IP address and a respondent id with each submission.
  5. Check Input fields to see exactly which answers are stored.
  6. Skip the Respondent's country block unless you really need country, as it adds a detail about where each person is.

Step 2: Switch off what could identify people

  • Respondent email notifications and email verification need an email address, so do not use them. Anonymous means no email.
  • Hidden fields such as ?user=123 link an answer to a person. Use them only for non-personal tags, such as a department or a channel.
  • Prevent duplicate submissions is useful, but check how you set it. An email-based check needs an email address, so pick an option that does not require one.
  • Personalized links (a different link per person) defeat anonymity. Use one link for everyone.

Step 3: Check the answers cannot give people away

Even with no names, answers can identify someone if the group is small.

  • Demographics in small teams. 'Department: Finance, role: Director, tenure: 10+ years' may point at one person. Use broader groups, make them optional, or drop them.
  • Free text. People may mention names or details. Tell them not to include identifying information, and read answers before sharing them.
  • Low response counts. If only three people answer, do not report by subgroup. Set a rule, such as not reporting any group under five people, before you start.

Step 4: Tell people what you collect

Put a short Text block at the top. Copy and adjust:

This survey is anonymous. We do not ask for your name or email and we do not store your IP address. Please do not include names or personal details in your comments. Results are shared only as group summaries. It takes about 3 minutes.

Only say what is true. If you did not check the settings above, do not make the promise.

Step 5: Share it without tracking people

  • Post one link where everyone can see it: an all-staff message, a notice board, a shared channel.
  • If you embed it on a page, do not use the option that saves the website page and query parameters unless you have checked what they contain.
  • Add the thank-you page text: 'Thank you. Your answers were saved without your name.'

Who can still see what

  • Workspace members can see the submissions. Check who is on the team under members.
  • The audit log shows who published, deleted, exported or changed what. That is a record of actions by your team, not of respondents.
  • Notification emails you receive contain the answers. If you forward them, you share the text.
  • Data retention can delete submissions after a number of days. Set it to match what you told people.

Honest limits: Qwenvo cannot make a survey anonymous when the questions are identifying, and it is hosted in the United States, so check whether your organization or country has rules about where survey data may live. Qwenvo has no HIPAA offering either. If the survey touches health information, ask your privacy officer first.

Questions people ask

Is an anonymous survey the same as a confidential one?

No. Confidential means you know who answered but promise not to reveal it. Anonymous means you do not know. Be clear which you offer.

Can I stop one person answering many times without collecting identifiers?

Not perfectly. Duplicate prevention helps, but any check that works on a person weakens anonymity. For a low-stakes poll, accept some repeats.

Can I make a survey anonymous and still analyze by group?

Yes, with broad, optional group questions and a minimum group size for reporting.

What if I need people to log in?

Then it is not anonymous. Use a normal form and say it is confidential. See how to write survey questions for fair wording, and the employee satisfaction survey template for a starting point.
