Order form template with Stripe payment: free, no platform fee

By the Qwenvo team · · 3 min read

An online order form needs a product list, a quantity, delivery details and a way to pay. Qwenvo's free Order Form template has all four, with a Payment block that charges through your own Stripe account. Qwenvo takes a 0% platform fee. Stripe's own fee still applies.

Fields in the template

  • Product: a dropdown with T-shirt at $25, Hoodie at $45 and Cap at $15. The prices are samples. Replace the items and prices with yours.
  • Quantity: a Number block. Customers type how many.
  • Full name and Email: the name goes on the parcel, and the email gets the confirmation.
  • Shipping address: one long answer box. Simple to start with, awkward to sort by country later.
  • Payment: the Payment block, connected to your Stripe account.

The thank-you page says "Order received! You will get a confirmation email." Turn on the confirmation email to respondents in Settings so that is true.

Improve it before you publish

  • Add an Image block under each product so people see what they are buying.
  • Use a calculated field for the order total, so the number the buyer sees matches what you charge. Test it with a one-item and a three-item order.
  • Split the shipping box into street, city, postcode and country, or add the Respondent's country block, so you can filter by region.
  • Add a Phone number block, optional, for delivery issues.
  • Add a "Notes for the seller" long answer for gift messages or sizes.

Copy-ready wording

  • Product: "What would you like to order?"
  • Quantity: "How many?"
  • Address: "Where should we send it? Include the country."
  • Notes: "Anything we should know? Size, colour or a gift message."
  • Thank-you page: "Thank you. Your order is in, and a confirmation is on its way. Shipping takes [your real number] days."

Logic ideas

  1. Show a size question only when the product is a T-shirt or Hoodie.
  2. Show a gift-message box only when the buyer ticks "This is a gift".
  3. Close the form on a date for a limited run, or after a set number of responses when stock is limited. The response cap counts orders, not units.
  4. Prevent duplicate submissions if you only sell one per customer.

What happens to the money

Payments go through your own Stripe account, so the money is yours and Stripe handles the card. Qwenvo does not add a fee on top. Stripe's own fee applies, so check Stripe's pricing page for what it charges you. Many currencies are supported. Before you go live, run a real test order from start to finish, including the confirmation email, so you know what a buyer sees.

For comparison, Tally also adds no fee on top of Stripe, so on this point the two are equal. The difference is what surrounds the order form: on Tally, your own domain, custom notification emails and the removal of the badge are Pro features at $29 a month.

Fulfil faster

Send each order to Google Sheets, Airtable or Notion as a row, or post a message to Slack or Discord. Use Zapier, Make or a webhook to feed a shipping tool. Export a day's orders as CSV or XLSX for your packing list. Per-submission PDFs make a handy packing slip for Latin-script names.

If customers also write notes or complaints, AI triage can tag urgent ones. See AI form triage.

Mistakes to avoid

  • Leaving the sample prices in place.
  • Never testing a payment from start to finish.
  • No shipping policy. Say how long it takes and what it costs before checkout.
  • A free-text quantity. Use a Number block so it cannot say "a few".
  • Selling more than you have, with no cap on responses.

One honest limit

Stripe is the only payment provider. If your customers need PayPal or a local method that Stripe does not offer in your market, an order form here is not the right tool. Jotform supports many processors; check their pricing page.
