Job application form template: fields, questions and AI triage

By the Qwenvo team · · 3 min read

A job application form should collect enough to decide who to talk to and nothing that slows good candidates down. Qwenvo's free Job Application Form template does that in two pages: contact details and a CV upload first, then experience, motivation and a start date. Edit the role, publish, and share the link.

What the template asks

Page one gathers the basics. The second page, titled About you, asks the questions you actually screen on.

  • Full name and Email: required. You cannot reply without them.
  • Phone (optional): many candidates prefer email first.
  • LinkedIn or portfolio URL (optional): a Link block that checks the format, so you get a clickable URL.
  • Upload your CV: a File upload block. Files can be up to 1 GB, so portfolios and work samples fit, where Tally's free plan stops at 10 MB per file.
  • Years of experience: a dropdown of 0-1, 2-4, 5-9 and 10+. Fast to read, hard to game.
  • Why do you want to work with us?: a long answer. This is the question that tells you who read the posting.
  • Earliest start date (optional): a Date block.

The progress bar is on in the template, which helps candidates see that the form is short.

Wording that gets better answers

  • Instead of "Why do you want to work with us?": "What about this role made you apply? Two or three sentences is plenty."
  • For the CV: "Upload your CV as a PDF. A portfolio link above is fine as well."
  • For experience: "How many years have you worked in a role like this?"
  • Thank-you page: "Application received. We read every one, and we will email you within [your real number] working days."

Add one role-specific question

A generic form attracts generic answers. Add one question tied to the work: "Describe the last project you shipped and what you owned on it." For a designer, ask for a link to a portfolio piece. For a developer, ask for a repository link. Use one such question, not five.

Logic ideas

  1. Add a hidden field called source and put it in the form link to learn which job board each applicant came from. See hidden fields.
  2. Add a yes/no "Are you legally able to work in this country?" question and jump a no to a polite end page.
  3. Show a cover-letter box only when the applicant picks "I want to add a note".

What to do with applications

Each submission lands in the Submissions table with the CV attached. With AI triage on, every application also gets a category, a priority, a sentiment and a short summary, for up to 10,000 submissions a month. That is a first-pass sort for a human, not a verdict. Do not reject anyone on an AI label. Read the answers, and check that your own criteria are fair before you use any ranking.

Routing rules can send high-priority applications to up to five email addresses, or to a Slack incoming webhook for the hiring channel. Export the table as CSV or XLSX when you need to share a shortlist. More in AI form triage.

Settings to switch on

  • Close on a date so the form stops when the role is filled.
  • Prevent duplicate submissions to stop repeat entries.
  • Confirmation email to the applicant so they know it arrived.
  • Data retention: auto-delete after N days, so old CVs do not sit forever. Check what the law in your country expects.

These live in the form Settings drawer. On Tally, data retention is a Business feature ($89 a month) and partial submissions are Pro.

Mistakes to avoid

  • Asking for date of birth, marital status or a photo. You rarely need them, and some are risky to collect.
  • Requiring a phone number. It drops some good candidates.
  • Making the cover letter mandatory.
  • Silence after applying. Send the confirmation and keep your promise on timing.
  • Collecting CVs with no plan for deleting them.

A fair limit

Qwenvo is a form builder, not an applicant tracking system. There are no pipeline stages or interview scheduling. Send submissions to Google Sheets, Airtable or Notion and track candidates there.
