How to Create an Order Form: Quantities, Totals and Payment

By the Qwenvo team · · 3 min read

To create an order form, list your items as questions with a Number block for quantity, add a calculated field that multiplies quantity by price and adds delivery, show the total, and take payment with a Payment block linked to your Stripe account. In Qwenvo all of that is free.

Plan the order before you build it

Write a table: item, price, anything the customer can choose (size, flavour), and the delivery options with their cost. Decide whether customers pay on the form or you invoice later. Keep the first version small. You can add items later.

Step 1: Add the items

  1. Click New form and name it, such as 'Spring bake sale order'.
  2. For each item, type / and add a Number block titled with the item and price, such as 'Chocolate loaf ($8 each)'. Leave it optional so customers can skip items they do not want.
  3. For choices like size or colour, use Multiple choice or Dropdown after the item.
  4. Add Short answer blocks for name, Email and Phone number.

Step 2: Calculate the total

Use a Calculated fields block to hold a number and change it with logic. The rules are in conditional logic.

  1. Insert a Calculated fields block and create a calculated field called total.
  2. Insert a Conditional logic block below the items. For each item, add a rule: Then Calculate, set total to quantity times price, or use the formula option.
  3. Keep the calculated block above the rules that change it, because rules run top to bottom and later rules win.
  4. Show the total to the customer with @total in a Text block: 'Your total is $@total'.

Test with a few quantities and check the maths by hand. The quote calculator guide goes further on calculated fields.

Step 3: Add delivery or pickup

  • Ask 'Delivery or pickup?' as Multiple choice.
  • If Delivery, use logic to show the address fields and add the delivery fee to total.
  • If Pickup, show a Text block with the pickup time and place.

Step 4: Take payment

Add a Payment block and connect your own Stripe account. Qwenvo adds a 0% platform fee; Stripe's own fee applies, so look up your rates on Stripe's pricing page. For payment details, see the Stripe payment form guide. If you would rather invoice, skip the Payment block and ask for a billing email instead.

Step 5: Confirm and track

  1. In Settings, switch on respondent email notifications and write a confirmation with @ mentions for the items and total.
  2. Switch on Self email notifications so each new order reaches you, with a PDF of the answers if useful.
  3. Write a Thank-you page with the pickup or delivery details.
  4. Set Limit number of submissions or Close form on a scheduled date to stop taking orders at your cut-off.
  5. Publish and share the link or embed it on your site.

Start from the order form template if you prefer, or the team lunch order and merch preorder templates for variations.

After orders arrive

  • Pick list: export Submissions as CSV or XLSX and sort by item.
  • Live view: send orders to Google Sheets or Slack with the integrations.
  • Packing slip: export a per-submission PDF for each order.

Mistakes to avoid

  • Hiding the delivery fee until the end. Show it as soon as the customer picks delivery.
  • Trusting the total without testing it. Place three test orders with different quantities.
  • No stock limit. Qwenvo does not track stock levels. Close the form or cap submissions when you run out.
  • Forgetting tax and shipping rules. Ask your accountant; the form only does the arithmetic you give it.

Honest limits: Qwenvo has no inventory management, product images in a shopping-cart layout or abandoned-cart emails. If you sell a large catalogue, a store platform is the better fit. For a bake sale, a lunch run or a small preorder, a form is enough. Stripe is the only payment provider. Will this stay free? If paid plans are ever introduced, everything you already use stays available and you will be told well in advance.
