To make a booking form, collect the service, preferred date and time, group size and contact details; add conditional logic for options that depend on the service; optionally take a deposit with a Stripe Payment block; and send a confirmation email. In Qwenvo you build it free by typing /.
First, be clear what this form does
A Qwenvo booking form takes booking requests. It does not hold a live calendar, check availability or block a time slot once someone picks it. That means you confirm each booking. For many small services, that is fine. If you need live slots, see 'Add live scheduling' below.
What to ask
- Service: a Multiple choice or Dropdown. Keep the names the same as on your price list.
- Preferred date and a second choice: two Date blocks. A backup date saves a round of emails.
- Preferred time: a Time block, or Multiple choice such as 'Morning', 'Afternoon', 'Evening'.
- Number of people or duration: a Number block.
- Contact details: Short answer for name, Email and Phone number. Their formats are checked as people type.
- Location or address: only if you travel to the customer.
- Special requests: a Long answer, optional.

Build it step by step
- Click New form and title it with the service, such as 'Book a studio session'.
- Type
/and add the fields above in the order a customer thinks: what, when, who. - Add conditional logic for service-specific questions. Example: only show 'How many rooms?' when the service is 'House cleaning'. See conditional logic.
- Add a Payment block if you want a deposit (see below).
- In Settings, switch on respondent email notifications, pick the Email question, and write the confirmation. Use
@to include their answers. - Switch on Self email notifications so you see each request.
- Write a Thank-you page that says what happens next and how fast you reply.
- Preview, test with your own details, and publish. Share the link or embed it from the Share tab.

Hi @Name, thanks for your booking request for @Service on @Preferred date. We will confirm within one working day. Your booking is not final until you hear from us.
Saying 'not final until we confirm' protects you from double-booking.
Add a deposit with Stripe
Type / and choose Payment, set the amount and currency, and connect your own Stripe account. Qwenvo charges a 0% platform fee; Stripe's own fee applies, so check Stripe's pricing page for your country. The setup is explained in the Stripe payment form guide. Stripe is the only payment provider. State your cancellation and refund terms in a Text block above the Payment block.
Add live scheduling
If people must pick from your real free slots, use a scheduling tool such as Calendly and drop it into the form with the Embed anything block, or collect the request first and send them the scheduling link on the thank-you page. Qwenvo has no built-in scheduling.
Control how many requests you get
- Limit number of submissions in Settings closes the form when the cap is reached.
- Close form on a scheduled date works for a seasonal offer or a launch.
- Closed form message tells latecomers what to do next, such as joining a waiting list.
Mistakes to avoid
- Treating a request as a confirmed booking. Say plainly that you will confirm.
- Asking for a time without a time zone if you serve people in different regions. Name the time zone in the question.
- Too many required fields. Require name, contact and date; leave the rest optional.
- No reminder. Add the booking details to your confirmation so customers have them.
Questions people ask
Can the form stop two people booking the same slot?
No. Qwenvo does not check availability. Confirm manually, or embed a scheduling tool that does.
Can I take payment when someone books?
Yes, through the Payment block with your own Stripe account.
Do customers need an account?
No. They open the link, fill it in and submit.
How do I see all my bookings?
In the Submissions table, or send each request to Google Sheets or Slack. See form to Google Sheets.
