How to Build a Lead Capture Form That Sales Can Act On

By the Qwenvo team · · 3 min read

A lead capture form collects the minimum a salesperson needs to follow up: usually name, work email and one qualifying question. Build it in Qwenvo with hidden fields for the traffic source, a Captcha against bots, and a routing rule that alerts you when a lead looks urgent. It is free.

Decide what a good lead looks like

Write down the two or three facts that tell you whether to call someone today or put them on a newsletter. Those become your qualifying questions. Everything else is friction. Test how many fields your audience will tolerate before you add more.

The fields

  • Name: Short answer. One field for full name is fewer taps.
  • Work email: Email block, required. It checks the format as people type.
  • Phone number: optional, unless you call every lead.
  • One qualifier: a Multiple choice such as 'What are you looking to do?' with three clear options plus 'Other'.
  • Timeline: Multiple choice: 'This month', 'This quarter', 'Just exploring'.
  • Consent: a required Checkboxes option saying how you will use their details.

Build it

  1. Click New form and title it with the offer: 'Get the pricing guide' or 'Book a demo'.
  2. Add the fields above with /. Put the easiest first.
  3. Type /hidden and add a Hidden fields block with utm_source, utm_medium and utm_campaign. Add the same names to your ad and email links, e.g. ?utm_source=newsletter&utm_campaign=autumn. Or embed on your site and use the Save website page and query parameters option so the landing page's tags carry over. See hidden fields and UTM tracking.
  4. Add a Captcha block to keep bots out. See contact form spam filter.
  5. Write a Thank-you page with the next step. If you promised a guide, link it here or use redirect on completion to send people to the download.
  6. Switch on Self email notifications so the lead lands in your inbox.
  7. Preview, then publish.

Start from the lead generation form, demo request form, content download gate or event lead capture template.

Copy-ready wording

  • Button label: 'Send me the guide' beats 'Submit'. Set it in Settings under Submit button label.
  • Consent line: 'I agree to be contacted about this request. I can opt out at any time.'
  • Thank-you: 'Thanks @Name. We will email you within one working day. In the meantime, here is the guide.'

Put it where leads are

  • A landing page, with a Standard embed or Full page embed. Paste the code into any builder that accepts HTML: WordPress custom HTML, Webflow, Framer, Wix, Squarespace or Notion's embed. See embed your form.
  • A popup with a delay or exit intent. See popup forms.
  • Your own domain, such as go.yoursite.com. See custom domains.
  • Ads and email, with UTM tags on each link.

Route hot leads fast

Speed matters more than polish for a warm lead. Turn on Triage submissions with AI in Settings. Each submission gets a category, a priority from low to urgent, a sentiment and a short summary, up to 10,000 a month. Add a routing rule: urgent or high priority goes to up to five email addresses or a Slack incoming webhook. Treat it as a first sort and review the rules after a week. More on this in lead scoring from form responses.

Send leads to where you work

  • Google Sheets, Airtable or Notion as a simple lead table.
  • Slack or Discord for instant alerts.
  • Zapier, Make or a webhook to push each lead into your CRM. Check your CRM's documentation for what it accepts.

Mistakes to avoid

  • Asking for everything up front. You can learn more on the call.
  • No source tracking. You will not know which campaign worked.
  • A dead-end thank-you page. Give the next step.
  • Letting leads sit. An unseen lead is a lost lead. Set up the alert before you launch.
  • Ignoring consent rules. They vary by country; Qwenvo is hosted in the United States, so check what applies to you.

Honest limit: Qwenvo captures and sorts leads; it is not a CRM. It has no pipeline view, deal stages or email sequences, so pair it with the tool where you manage deals.

Questions people ask

How many fields should a lead capture form have?

As few as your follow-up needs. Start with name, email and one qualifier, then test adding more.

Can I stop spam leads?

Yes. Use the Captcha block, and the AI triage spam category helps you sort what gets through.

Is a lead capture form free?

Yes. Tally puts the branding removal, custom domains and analytics many lead pages need on its paid plans; on Qwenvo they are free. See Qwenvo vs Tally.
