Teachers can make free surveys on Qwenvo for end-of-term feedback, exit tickets, parent surveys and permission slips. Students and parents need no account, forms live in folders per class, and a retention setting deletes old responses automatically. There are no paid plans.
Five classroom jobs, one tool
- End-of-term feedback. The student feedback survey uses a matrix of statements such as "Lessons were clear" on an agree scale, an overall rating, and two open questions: what helped most and what should change.
- Exit tickets. One to three questions at the end of a lesson. Build a three-question form once and reuse the link each day.
- Permission slips. The field trip permission slip collects student, grade, guardian, emergency phone, medical needs and a guardian signature in a Signature block.
- Club sign-ups. The after-school clubs template lists clubs, days and how the child gets home.
- Peer observation. The classroom observation form rates objectives, engagement and behaviour on a four-level scale with strengths and next steps.
Set up a survey in ten minutes
- Create a free account with your school or personal email.
- Open a template, or describe what you need to the AI on the empty-form screen and edit the draft.
- Cut it to what you will actually read. If you would not act on an answer, delete the question.
- Add a Text block that says how long it takes and whether answers are anonymous.
- Publish and copy the link from the Share tab. Paste it into your class page, learning platform or email.
- Check Submissions after the first few answers to make sure the data looks right.
Keep classes organized
Qwenvo has unlimited workspaces and folders and unlimited teammates, so you can set up a folder per class or per year and add a colleague to help with a shared survey. See members. The audit log records who published, deleted or exported what, and 90-day version history lets you roll back a bad edit.
Surveys students will answer honestly
- Say plainly that the survey is anonymous, then do not ask for names. In the Settings, you can choose which fields are stored.
- In a small class, even a "which period are you in" question can point to one student. Ask only what you need.
- Keep rating scales short and labelled.
- Allow a blank answer for open questions, so people are not forced to invent something.
- Show what you changed afterwards. Students answer honestly when they see feedback lead somewhere.
Protect student information
Use Submissions data retention to delete responses automatically after a number of days, and Password protect form for staff-only forms. Qwenvo is hosted in the United States and has no HIPAA offering. Check your school or district policy before collecting medical or other sensitive information, and do not collect more than a form needs. See the privacy page.
Read the results
Insights shows charts and averages for each question. Export CSV or XLSX for your own spreadsheet, or connect Google Sheets so responses arrive live. For a feedback survey with open comments, Triage submissions with AI can give each comment a sentiment and a short summary. It suggests, you decide, and you should read the comments themselves before drawing a conclusion.
Where Google Forms is the better pick
If your school runs on Google accounts, Google Forms sits inside the tools you already use and has a built-in quiz mode with grading, which makes it quick for a simple graded quiz. Qwenvo is the stronger choice when you want branding control, no sign-in for students (Google Forms needs one to upload files), calculation logic and AI summaries.
A simple routine for the term
- Week one: a short "about you" form for new students or parents, using only questions you will read.
- Each unit: a three-question exit ticket using the same link, so results build up in one Submissions table.
- Midway: a quick pulse survey with one rating and one open question.
- End of term: the student feedback survey, then a short note to students on what you will change.
Reusing one form per purpose means one place to look, and Insights charts show how answers move over the term.
