Free contact form template: fields, wording and spam tips

By the Qwenvo team · · 4 min read

A good contact form asks four things: who you are, how to reply, what the message is about and the message itself. Qwenvo's free contact form template has exactly those: a name, an email, a "What is this about?" dropdown (Sales, Support, Partnership, Other) and a message box. Open it, change the topics to match your business, and publish.

Use the contact form template. It is free, needs no card and takes about two minutes to adapt.

The fields in the template

  • What is your name? A short text answer. You need it for the greeting in your reply, nothing more.
  • What is your email? An email field, so a mistyped address is caught before it is sent.
  • What is this about? A dropdown with Sales, Support, Partnership and Other. This is the field that saves you time, because it tells you who should read the message before anyone opens it.
  • Your message. A long answer box. The template's intro promises a reply within one business day, so change that line if it is not true for you.

Resist adding more. Every extra required field costs you some senders. A phone number is the usual temptation. If you do want one, add a Phone number block and make it optional.

Question wording you can copy

  • Instead of "Subject": "What can we help with?"
  • Instead of "Message": "Tell us a bit more. The more detail you give, the faster we can answer."
  • Instead of "Name": "What should we call you?"
  • For the thank-you page: "Got it. A real person reads every message, and we reply within one business day."

Logic worth adding

Type / and add conditional logic, or use "Add conditional logic" on a question's menu. Two rules cover most sites:

  1. If the topic is Support, show an extra question: "Which product or page is this about?" Support messages with no context are the slowest to answer.
  2. If the topic is Sales, show "Company name" and a Phone number block. Sales leads are worth the extra friction. General questions are not.

Keep it light. Learn how rules work in the conditional logic guide.

What to do with the messages

In the form Settings drawer you can switch on email notifications to you and a confirmation email to the sender. Turn both on. Then decide how much you want to read by hand.

Qwenvo's AI triage gives every submission a category, a priority (low, normal, high or urgent), a sentiment, a one-to-two-sentence summary and a suggested reply, for up to 10,000 submissions a month. Add a routing rule: urgent goes to a Slack channel, partnership goes to the person who owns it. Read more in AI form triage.

If you live in a spreadsheet, connect Google Sheets. If your team uses Notion, Airtable, Discord or Linear, those are built in too, and Zapier, Make and webhooks cover the rest.

Spam and other mistakes

  • Publishing a bare email address instead of a form. Addresses on a page get scraped. A form with a Captcha block does not expose yours.
  • No Captcha. Add the Captcha block before the form goes on a busy page. If spam still gets through, turn on respondent email verification, which sends a one-time code.
  • Too many topics in the dropdown. Four is plenty. People pick "Other" when the list is long.
  • A vague promise. "We will get back to you soon" tells nobody anything. Say a number of days you can keep.
  • Forgetting to test the notification. Submit the form yourself and check the email arrives.

Put it on your site

Open the Share tab and copy the Standard, Popup or Full page embed. Paste it into any builder that accepts HTML: a WordPress custom HTML block, a Webflow or Framer embed, a Wix or Squarespace code block, or Notion with /embed. See embedding a form. To use your own address such as contact.yourdomain.com, add a custom domain.

On Tally, switching off the badge, using your own domain and writing custom notification emails sit on Pro, which is $29 a month. Here they are part of the free account. See Qwenvo vs Tally.

One honest limit

Qwenvo has no mobile app, so you read messages by email, Slack or the web dashboard. If you want a push notification on your phone, route urgent triage results to a Slack channel and use Slack's app.

Questions people ask

How many fields should a contact form have?

As few as you can reply with. Name, email and message are the minimum. A topic dropdown is the one extra field that earns its place.

Can I use this contact form template for free?

Yes. Every template is free and every account has every feature. There is no plan to pick.

How do I stop spam on a contact form?

Add a Captcha block first. If that is not enough, require respondent email verification so each sender confirms an address with a one-time code.

Every feature, $0, no card.
