Webinar registration form template with UTM tracking

By the Qwenvo team · · 3 min read

A webinar registration form needs to get an email address, learn who is coming and record where they heard about it. Qwenvo's free Webinar Registration template does that with seven questions and two hidden UTM fields, so you see which campaign each registrant came from without asking.

The fields in the template

  • Hidden fields utm_source and utm_campaign: filled from the link and never shown. They tell you which email, post or ad produced each registrant.
  • First name and Last name: two fields, so you can greet people properly.
  • Work email: an Email block. Asking for a work address helps qualify B2B registrants.
  • Company: a short answer.
  • Job role: a dropdown of Marketing, Sales, Product, Founder and Other.
  • A question for the speakers (optional): a long answer. Questions collected ahead let you shape the talk.
  • Send me the recording: a Yes or No choice, so people who cannot attend still register.

The intro says "Live on Thursday at 4 pm CET. A recording is sent to everyone who registers." Replace it with your real time, and only promise a recording if you will send one.

How the UTM tracking works

Add ?utm_source=newsletter&utm_campaign=october-webinar to the form link you share in each channel. The hidden fields store those values with the submission. Count registrants by source in Submissions or in a sheet. Google Analytics 4 and Meta Pixel can be added to the form too, if you run ads. Setup is in the hidden fields guide. On Tally, GA and Meta Pixel need Pro. Test the link yourself before you send it: submit once and check that both values appear in the row. A typo in the parameter name is the most common reason the columns stay empty.

Copy-ready wording

  • Headline: "Save your seat." Add one line on what attendees will learn.
  • Role: "What best describes your role?"
  • Speaker question: "What would you like the speakers to cover? We read every question."
  • Recording: "Cannot make it live? Send me the recording."
  • Thank-you page: "You are registered. Check your inbox for the confirmation and calendar details."

Logic ideas

  1. If the role is Founder, show "How many people are in your team?"
  2. If the role is Other, show a short answer for the role name.
  3. If the role is Sales or Marketing, show "Which tools do you use today?" so you can tailor examples in the session.
  4. Close the form on the webinar's start time, or after a set number of responses if your webinar room has a seat limit.

After someone registers

Turn on the confirmation email to respondents in form Settings, so each person gets a receipt straight away. Qwenvo does not host the webinar or send reminders. Push each registrant into your webinar tool or email platform with Zapier, Make or a webhook. Or send rows to Google Sheets and import them.

Add your call-to-action after the form. Use "redirect on completion" to send people to a page with calendar links or a welcome video.

Use the speaker questions

Questions are the best content planning you will get. Export them, or let AI triage group them: each response gets a category and a one-to-two-sentence summary, for up to 10,000 submissions a month. Read the summaries to find the three themes that appear most, and build the session around them. Mention one or two questions by name on the day. People who see their question answered stay to the end.

Mistakes to avoid

  • Asking for a phone number. It lowers registrations and rarely helps.
  • Hiding the date and time. Put it at the top.
  • No confirmation email or calendar details. No-shows climb.
  • Promising a recording and then not sending it.
  • Not testing the UTM link before you send it to your list.

Limits to know

Qwenvo has no built-in scheduling or calendar invites. To let people pick a time slot, embed Calendly with the Embed anything block. It also has no built-in QR code, so paste the link into any QR generator if you need one for a slide.
