A restaurant feedback form should take thirty seconds, fit on a phone and tell you what went wrong while the guest is still at the table or just left. Qwenvo has no restaurant-specific template. The quickest route is to adapt the free Customer Feedback Form and swap in the questions below.
Start from the template and change it
The base template has an overall star rating, a 0-10 recommend scale, a checkbox list of what they liked, an open box for improvements and an optional email. For a restaurant, keep the star rating and the open box, change the checkboxes and add a few blocks. The edits take about ten minutes, and the changes below are our suggestions, not part of the original template.
Fields to use
- Overall experience: keep the five-star rating from the template.
- Rate each part of your visit: a Matrix block with rows for Food, Service, Atmosphere and Value, and columns from Poor to Excellent. One question, four answers.
- What did you order? (optional): a short answer, or a dropdown of your menu sections. Helps you link complaints to dishes.
- Visit type: Dine-in, Takeaway or Delivery. A takeaway complaint and a dine-in complaint need different fixes.
- How likely are you to recommend us?: keep the 0-10 scale from the template if you want an NPS figure.
- What could we do better?: the open box. The most useful field on the form.
- Email (optional): for the reply. Keep it optional.
- A hidden field called table: pass the table number in the link, so feedback ties to a table or a server without asking the guest.
Wording for the dining room
- "How was everything today?" instead of "Rate your overall experience".
- "Was there anything we got wrong? We want to fix it."
- "Want us to follow up? Leave your email."
- Thank-you page: "Thank you. The manager reads every response." Only if the manager does.
Logic worth adding
- If any rating is low, show "Tell us what happened" as a required question.
- If the visit type is Delivery, show "Was your order complete and on time?"
- If the overall rating is five stars, show a Link block pointing to your public review page, so only happy guests are nudged to post one.
- If the visit type is Dine-in, show the table question. Takeaway guests have no table to name.
Catch problems the same day
Turn on AI triage in the form Settings. Every response gets a category, a priority (low to urgent), a sentiment, a short summary and a suggested reply, for up to 10,000 submissions a month. Add a routing rule that sends high and urgent priorities to the manager by email (up to five addresses) or to a Slack incoming webhook. A food safety complaint should never wait for a weekly review. Triage helps you spot it. A person still decides what to do.
For trends, send rows to Google Sheets and track the Food, Service and Atmosphere averages week by week. Insights also charts averages and NPS.
Getting the link to diners
Print a QR code on the receipt or table card. Qwenvo has no built-in QR code, so make the form, copy its link and paste it into any QR generator. Use a different link per location or table with a hidden field. You can also put the link in your receipt email or your Instagram bio. If you want your own address such as feedback.yourrestaurant.com, add a custom domain, free here.
Mistakes to avoid
- A long form at the end of a meal. Five or six questions is the limit.
- Asking for a name and phone number up front.
- Reading feedback only when something goes wrong. Praise tells you what to protect.
- No reply. A guest who leaves an email expects an answer.
- Offering a discount in the form without a plan to track and honor it.
An honest limit
Qwenvo has no mobile app, so staff read feedback by email, Slack or the web dashboard. And a form does not post to Google or Yelp for you; it only collects private feedback that you decide what to do with.
