Quote request form template: questions that qualify leads

By the Qwenvo team · · 3 min read

A quote request form should give you enough to price the job without a call: what the person wants, how big it is, what they can spend and when they need it. Qwenvo's free Quote Request Form template asks eight questions that qualify a lead and keep it to one page.

The fields in the template

  • Name and Email: required. Your quote goes to this address.
  • Phone (optional) and Company (optional): helpful context. A company name often tells you the size of the job.
  • Service: a dropdown with Website, Branding, SEO, Paid ads and Content. Replace it with your own services.
  • Describe the project: a required long answer. The most important field on the form.
  • Budget: Under $2k, $2k-10k, $10k-25k or $25k+. Edit the bands to match your prices.
  • When do you want to start?: ASAP, Within a month, In 1-3 months or Just researching.

The thank-you page promises a quote within two business days. Change it to a promise you can keep.

Why budget and timing are on the form

Asking for a budget feels awkward, so many forms skip it, and then you spend a week quoting a client who can only afford a tenth of it. A range is easy to answer and lets you decide in seconds which requests deserve a custom proposal, which get a standard package, and which get a polite no. "Just researching" tells you to send information, not a formal quote.

Copy-ready wording

  • Project: "Tell us what you want to achieve. A few sentences and any links are plenty."
  • Budget: "Which range is closest to what you have in mind? A rough range helps us suggest the right option."
  • Timing: "When would you like the work to begin?"
  • Thank-you page: "Thanks. We will send your quote within two business days. If it is urgent, say so in your message."

Logic ideas

  1. If the service is Website, show "Do you already have a site?" and a Link block for its address.
  2. If the budget is the lowest band, show a note about your minimum project size, or jump to a page with a lighter option.
  3. If the start time is ASAP, show "What is driving the deadline?" so you can judge urgency.
  4. Add a File upload block for briefs or reference files. Up to 1 GB per file.

For prices that depend on options, use a calculated field to build an estimate. See the conditional logic help article.

Route the hot leads first

Turn on AI triage. Each request gets a category, a priority, a sentiment, a short summary and a suggested reply, for up to 10,000 submissions a month. Write a routing rule: high and urgent priorities go to the person who closes deals, by email (up to five addresses) or a Slack incoming webhook. Lower priorities wait for your weekly batch. The suggested reply is a draft for you to edit, not a quote.

Then send every request to your CRM-like sheet in Google Sheets, Airtable or Notion, or push it through Zapier, Make or a webhook. Read more in AI form triage.

Settings to switch on

  • A confirmation email to the requester, so they know the form worked.
  • Email notifications to you, so a new request does not wait in a dashboard.
  • Respondent email verification (a one-time code) if spam is a problem. On Tally that sits on the Business plan at $89 a month.
  • A Captcha block for public pages.

Mistakes to avoid

  • Too many questions. Eight is plenty. Every extra field loses real leads.
  • Making the budget optional. You will quote blind.
  • A slow reply. Speed is often what wins the job, so set up notifications.
  • Promising a quote time you cannot meet.
  • Treating a lead's budget range as a negotiation. It is information.

A fair note on scope

This is a form, not a proposal or invoicing tool. It collects the request, and you write and send the quote yourself. Qwenvo also has no built-in scheduling. To let a lead book a call straight after submitting, add Calendly with the Embed anything block.

Questions people ask

What should a quote request form include?

Contact details, the service wanted, a project description, a budget range and a start date. Those five cover most jobs.

Can the form show an automatic price?

Yes, if your pricing is simple enough. Calculated fields can add up prices from the answers. Test the numbers carefully.

Can I put the form on my website?

Yes. Copy the Standard, Popup or Full page embed from the Share tab. See embed help.
