To build a free registration form for a class, club or race, ask who is taking part, collect any consent or emergency details, cap the numbers, and optionally add a Stripe payment. Qwenvo has templates for each, a form-level limit on submissions, and no fee from us.
Choose the template that matches your group
- [Registration form](/templates/registration-form): general purpose. Name, email, optional phone, a ticket type, sessions as checkboxes and dietary requirements.
- [After-school club sign-up](/templates/after-school-club-signup): student name, grade, which clubs, which days, how the child gets home and a parent email and phone.
- [Fun run registration](/templates/fun-run-registration): distance, T-shirt size, date of birth, emergency contact, then a waiver signature and an entry fee on a second page.
- [Course enrollment](/templates/course-enrollment): student, course, schedule, previous experience and a tuition payment.
What registrations should collect
The art is to ask the minimum that lets you run the activity safely.
- Participant details: name, and age or grade if it affects grouping.
- A contact who will answer: email, plus a phone number for last-minute changes.
- Emergency contact for anything physical.
- Medical or access needs, phrased as "anything we should know" rather than as a diagnosis.
- Consent or waiver, with a Signature block.
- Choices: session, level, size, day.
When the participant is a child, make the form addressed to the parent or guardian and say so in the title. Collect only the child's details you need.
Build it
- Create a free account and start from a template.
- Rewrite the title and intro with the date, time, place, age range and cost.
- Edit the choices to match your real sessions and options.
- Use Add conditional logic to show follow-ups only when needed, such as a medical details box only if the answer is yes.
- Add a waiver as a Text block followed by a Signature block.
- Turn on the Progress bar if the form has several pages.
- Set confirmation and notification emails in Settings.
- Publish, then share the link or embed it on your club's website.
Cap the numbers
- Limit number of submissions closes the form after a total is reached. It counts registrations, so if one form can register several people, count carefully.
- Close form on a scheduled date shuts registration the day before the start.
- Closed form message should say whether there is a waitlist. The workshop waitlist template can collect names for one.
- Prevent duplicate submissions reduces double sign-ups.
Charge a fee if you need to
Add a Payment block to the end and connect your own Stripe account. Enter the fee in Price and pick the currency. Qwenvo takes 0% and Stripe's own fee applies. If fees depend on options, such as a member price and a guest price, use a calculated field and the picker beside Price. See free event registration with payments.
Keep track of who is in
Submissions is your roster. Sort, search and export it to CSV, JSON or XLSX, or let it flow into Google Sheets or Airtable. A coach can export on the day and print the list. For the first session, a quick check-in form like the event check-in template can record who showed up.
Protect participants' information
Use Submissions data retention to auto-delete registrations after the season. Use a password for a members-only form. Keep emergency and medical details to the minimum. Qwenvo is hosted in the United States and has no HIPAA offering, so do not collect clinical health records.
Wording for the top of the form
The first lines decide whether people read the rest. State what it is, when it starts, who it is for and the cost, then say what happens next. For example: "Autumn football for ages 8 to 11, Saturdays from 9 October. Register below and we will confirm your place by email within three days." Replace the details with your own, and never promise a place if a coach still has to approve it.
Mistakes to avoid
- Collecting a birth date when an age group would do.
- Making the medical question required for everyone, instead of asking "anything we should know?"
- Forgetting to test the confirmation email with a real address.
- Leaving the form open after the class is full.
Questions people ask
Can I register several people in one submission?
Yes, with a Number block for how many, and extra fields for names. Keep it simple, or use one submission per person if you need a clean roster.
How is this different from the event registration guide?
That one covers a conference-style event. This one covers the recurring activity with ongoing members and parents. See also how to make an online registration form.
Is it free?
Yes. There are no paid plans.
Every feature, $0, no card.
