Free form builder for Shopify: embed a Qwenvo form on a page

By the Qwenvo team · · 3 min read

To add a free custom form to Shopify, build it in Qwenvo, copy the Standard embed code from the Share tab, and paste it into the HTML view of a Shopify page. Shopify keeps selling your products; the form handles what checkout does not, like applications, custom requests and waitlists.

When a form helps a Shopify store

Shopify has its own contact form and checkout. Use a separate form when a customer needs to tell you something that checkout has no field for:

  • Wholesale and partner applications. Collect company, website, partnership type and how they would sell your product. The partner program application template is a starting point.
  • Custom order or quote requests. Use the quote request form: service, description, budget and timeline.
  • Back-in-stock and launch waitlists. The product waitlist template collects email, use case and where they heard of you.
  • Product feedback after delivery. See the customer feedback form.
  • Event or workshop sign-ups you host in store.

Build the form

  1. Create a free account and open a template that matches the job.
  2. Match your store: set colours and fonts in Customize, add your logo, and switch off the Qwenvo branding badge in Settings.
  3. Set notifications to go to the person who handles the requests, and write a confirmation email.
  4. Publish the form.
  5. Open the Share tab and choose Standard. Copy the snippet.

Put it on a Shopify page

  1. In your Shopify admin, add a new page (for example "Wholesale application") from the online store pages.
  2. In the page's content editor, switch to the HTML view, which is usually the "<>" button.
  3. Paste the Qwenvo embed code and save.
  4. View the page on your store, submit a test response and check that it arrives.
  5. Add the page to your menu or footer.

Shopify's admin changes over time, and themes differ, so if you cannot find the HTML view, check Shopify's help center for your theme. The same snippet works in any block that accepts HTML. If the form does not appear, you probably pasted into a plain text area. More in embed a form on a website.

Make it feel like part of the store

  • Use a custom domain such as forms.yourstore.com. Add it on the Domains page, create a CNAME at your DNS provider, click Verify and pick it on the Share tab. See custom domains.
  • Turn on Transparent background and Hide title in the embed options so the form blends into the page.
  • Use Popup for a waitlist that opens on a button or after a delay. See popup forms.

Get the data where you work

Responses land in Submissions, and you can export CSV, JSON or XLSX. Send them to Google Sheets, Airtable, Notion or Slack, or through Zapier, Make or a webhook to other tools. There is no dedicated Shopify integration, so any link to your orders or customers goes through those general connectors.

Payments: keep products in Shopify

Sell your catalog through Shopify checkout. Use Qwenvo's Payment block for things outside it, like a deposit for a custom job or a paid workshop. It uses your own Stripe account, charges a 0% platform fee and leaves Stripe's own fee in place. Stripe is the only provider on Qwenvo.

An honest limit

A form is not a store feature. It does not read your product catalog, inventory or order history, and it cannot attach a request to a Shopify customer record on its own. If you want something that lives inside your Shopify admin, look at Shopify's own tools and apps. If you want a flexible form that is free and works on any site, embedding fits well.

Test before you launch

  • Open the page on a phone and a laptop and make sure the form fits without side scrolling.
  • Submit a test response and confirm the notification email, the confirmation email and any integration all fire.
  • Check the thank-you message tells customers what happens next and how long it takes.
  • If you use a popup, make sure it does not cover the cart or checkout buttons.

Questions people ask

Is there a Shopify app for Qwenvo?

No. You embed the form with a snippet, which needs no app.

Can I put the form on a product page?

Wherever your theme lets you add an HTML or custom code block, yes. Where it does not, link to the form page.

Does it cost anything?

Qwenvo costs $0. Shopify's own plan and Stripe's fees are separate. See Qwenvo vs Tally if you are comparing form tools.
