To create a feedback form online for free, choose the moment you want feedback about, ask a rating plus one open question, and put the form where people already are: a link after purchase, a popup on your site or an email. Qwenvo includes the charts and AI sorting, with no paid plan.
Decide the moment before the questions
A feedback form that works for an event will fail on a website. Pick one of these and start from the matching template:
- After a purchase or support chat. The customer feedback form asks for a five-star rating, a 0 to 10 recommend score, what they liked as checkboxes, what to improve, and an optional email if they want a reply.
- On your website. The website feedback template is only two questions: did you find what you were looking for, and what were you looking for. Short forms get answered on busy pages.
- After an event. The post-event feedback template rates keynotes, workshops, venue, food and networking in one matrix, then asks for a favourite moment and what to improve.
A four-question structure that works
- One rating that gives you a number to compare over time: stars or a 0 to 10 scale.
- One reason question, placed right after the rating: "What is the main reason for your score?" The number says how they feel; the sentence says why.
- One specific question about the thing you can change, such as a multiple choice of features.
- One optional contact question, only for people who want a reply.
Do not make the open question required. A required open question produces junk text. Do make the rating required, so you can chart every response.
Build and brand it
- Create an account and open your template.
- Cut questions until you are down to the four above, then reword each to be about one thing.
- Type
/to add a Rating or Linear scale block where you need a number. - In Customize, add your logo and colours. In Settings, switch off the Qwenvo badge.
- Publish and test it on a phone.
Put it where people will answer
- A link in a receipt or follow-up email. Add a hidden field such as an order number so each reply is tied to a customer without asking for it. The NPS survey template uses hidden fields for exactly this.
- A popup on your site. From the Share tab, choose Popup and trigger it by delay, scroll depth or exit intent. See popup forms.
- A redirect. Use Redirect on completion on an order form to send buyers to your feedback form.
- A full-page link printed on a QR code at a counter. Qwenvo has no built-in QR generator, so paste the link into any QR tool.
Read the replies without drowning
Open Insights for charts, averages and the NPS score. Open Submissions for the text. Switch on Triage submissions with AI in Settings and each reply gets a category, a priority, a sentiment, a one-or-two-sentence summary and a suggested reply. Add a routing rule so urgent or negative replies go to a Slack webhook or up to five email addresses. See AI form triage.
The AI suggests; you decide. It reads one reply at a time and will not write a themed report across thousands of responses, so skim its summaries, then read the originals before you quote or act.
Mistakes to avoid
- Asking leading questions such as "How great was your visit?"
- Mixing two topics in one question, like "staff and prices".
- Forgetting a way to say nothing was wrong. Add a "Nothing, all good" option to checkbox lists.
- Never acting on the answers. Close the loop by telling people what you changed.

Yes, if you do not ask for a name or email. Check your Settings: Qwenvo can store an IP address and respondent id with each submission, and you can choose which fields are stored.
How many responses can I collect free?
There is no response cap on the free account, within the fair use policy.
Can I translate the form?
The form's own buttons and messages can be shown in 45+ languages, including right-to-left languages. Your questions are the text you write.
