A customer feedback form works when it takes under a minute and asks one thing you can act on. Qwenvo's free Customer Feedback Form template has five questions: a star rating, a 0-10 "would you recommend us" scale, what they liked, what to improve, and an optional email so you can reply.
The five questions, and why each is there
- Rate your overall experience: a five-star Rating block. It is the fastest thing to answer, so almost everyone does.
- How likely are you to recommend us to a friend?: a Linear scale from 0 to 10 labeled Not likely and Very likely. This is the Net Promoter Score question and it feeds the NPS figure in Insights.
- What did you like?: optional checkboxes: Speed, Quality, Price and Support. Replace them with the things your customers actually choose you for.
- What could we improve?: an optional long answer. This is where the useful sentences are.
- Email (optional, if you want a reply): optional on purpose. Anonymous feedback is more honest, and people who want an answer will give an address.
The template ends on "Thank you for your feedback!". Change it to say what you will do with it.
Copy-ready wording
- Rating: "How was your experience with us today?"
- Recommend: "How likely are you to recommend us to a friend or colleague? 0 means not at all, 10 means definitely."
- Liked: "What stood out? Pick any that apply."
- Improve: "If you could change one thing, what would it be?"
- Email: "Want a reply? Leave your email. Otherwise leave it blank."
Logic that makes the form smarter
The template has no rules, but two small ones make it better.
- If the star rating is low, show "What went wrong?" as a required question, and make the email prompt more prominent. Unhappy customers are the ones you can still save.
- If the rating is high, show a Link block asking them to leave a public review. Send happy people where you want them to go.
Rules are covered in conditional logic.
What to do with responses
Open Insights to see charts, averages and the NPS score. For the free-text answers, turn on AI triage. Every submission gets a category, a priority, a sentiment, a short summary and a suggested reply, for up to 10,000 submissions a month. Add a routing rule so negative, high-priority feedback goes straight to a person by email (up to five addresses) or into a Slack incoming webhook. Everything else can wait for a weekly read.
For reporting, send answers to Google Sheets, Notion or Airtable, or use Zapier, Make and webhooks. Our post on AI form triage walks through routing.
Mistakes that ruin feedback forms
- Asking too much. Past five or six questions, completion drops. Cut anything you will not act on.
- Forcing an email. Required emails push people to leave or to type nonsense.
- Leading questions. "How great was our service?" gets you praise and no information.
- Never reading the free text. Ratings tell you that something is wrong. The comments tell you what.
- Not closing the loop. Reply to the people who left an email. It is the cheapest loyalty move you have.
Where to send the link
Put it in the receipt email, the end of a support chat or on the page after checkout. Share it as a link or embed it from the Share tab. If you want a clean look, switch off the Qwenvo branding in Settings, which is free. Tally's badge removal needs Pro, which is $29 a month.
Track where feedback comes from
Add a hidden field such as channel or order_id and pass it in the link. You then see which channel or order each answer came from without asking the customer. See hidden fields.
A fair note
Qwenvo launched in October 2026, so it has far fewer public reviews than Tally, which has been around since 2021. The template and the features are yours to test for free before you rely on them.
